Services

Clinical Nutrition Case Review and Nutritional Assessment

A comprehensive clinical dietetic assessment designed to identify nutritional factors that may be limiting recovery.

An assessment is always required before recommendations can be made or intervention is given but can also be offered as a one-off to access immediate dietetic advice, to receive a report of recommendations for nutrition support or an estimate of input that will be needed.

  • suspected malnutrition or inadequate intake 

  • Muscle weakness, loss of muscle mass and weight loss as a result of serious injury.

  • Previous ICU admission (within the past 12 months)

  • Reduced food intake

  • Swallowing problems

  • fatigue and/ or gut symptoms impacting nutritional intake and rehabilitation 

  • Reduced exercise tolerance

  • complex or multi-factor presentations or cases where progress is unclear or stalled  

Refer when:

  • review of referral supporting documentation, medical history, dietary intake, available blood results, health data, social and environmental factors & activity levels

  • assessment of nutritional status and risk 

  • identification of contributing factors 

  • detailed report with clinical recommendations 

Includes:

Targeted Nutrition Intervention and Management

Focused dietetic intervention and management to address identified issues and support recovery.

  • restoring nutritional adequacy through dietary advice, nutrition planning and education 

  • managing symptoms affecting intake 

  • implementing supplementation strategies where appropriate 

  • supporting & optimising long term health conditions and quality of life aspects

  • practical support and meal planning

  • focus on client-specific goals and outcomes

All input whether sessional or in a programme of intervention includes:

  • regular review and adaptation of nutrition strategy 

  • support through different stages of recovery 

  • collaboration with the multidisciplinary team 

  • outcome monitoring and reporting

Nutrition strategies are personalised and aligned with rehabilitation goals and level of complexity. Intervention is typically offered in the form of a programme of intervention ranging from a few targeted sessions, or 6-12 weeks of structured input. 

Input is adapted to the level of complexity and recommendations will be made on a case by case basis.

Nutrition Rehabilitation Programmes

Fuel-to-Function programme

12 Week Signature Programme

The Fuel to Function™ programme is a structured nutrition rehabilitation pathway designed to support recovery after traumatic injury or complex illness.

The programme takes place over approximately 12 weeks and provides comprehensive dietetic intervention for individuals with complex nutritional needs or where longer-term health and recovery goals are being addressed, not only through dietary advice, but through supportive practical and health coaching strategies.

  • addressing symptoms that affect intake and recovery

  • supporting rehabilitation tolerance and participation in therapy

  • building practical nutrition strategies that support longer-term health and independence, functional goals and quality of life.

Dietary changes are supported through practical as well as long term behavioural adjustments and take time to embed. Throughout the programme, nutrition strategies are adapted to the individual’s recovery stage and rehabilitation goals.

Foundations programme

6 Week Programme

The Foundations programme is designed to stabilise nutritional status and address early barriers that may be affecting recovery. It focuses on supporting adequate nutritional intake, managing common symptoms that interfere with eating and providing practical strategies that help individuals participate more effectively in rehabilitation and daily activities.

  • management of appetite disruption and fatigue

  • practical meal strategies to support day-to-day functioning

  • improved rehabilitation tolerance

The programme typically supports:

This programme is often suitable for individuals with lower complexity nutritional needs, or as an initial phase of intervention before progressing to a longer programme if required.

Sessional Dietetic Input

For some clients, targeted sessional dietetic input may be sufficient without a full programme. This option provides a small number of structured consultations focused on assessment, education and practical dietary guidance. It may be appropriate where a specific nutrition concern has been identified and short-term input is likely to be sufficient.

  • liaison with the wider rehabilitation team where appropriate

Includes:

It is recommended that a client has at least 2-3 sessions for appropriate follow-up, monitoring and reporting to occur. This is most suitable for a client who simply needs some nutrition education and is able to self-manage. Specific outcomes around dietary or health changes cannot be monitored or guaranteed.

Working with Case Managers and Teams

  • recovery is slower than expected 

  • fatigue limits participation in therapy 

  • nutritional intake is reduced or inconsistent 

  • malnutrition, weight or muscle loss is a concern 

  • there is uncertainty around the role of nutrition in recovery

Our role is to provide clinical clarity and targeted strategies that support rehabilitation outcomes.

  • A report after assessment with clear recommendations

  • Input to the client of 1:1 consultations every 2 weeks with Whatsapp or email support in between for questions. (Clients on a programme of 6 weeks, will have 3 x 1:1 consultations, and clients on a 12 week programme will have 6 1:1 consultations)

  • Practical tools, handouts, audio or video educational materials

  • Objective outcome-based monitoring

  • Liaison with the MDT and case manager

  • Mid-way report (12 week programme only)

  • A final report (for all programmes/ after final session).
